Modality: The DOS team will schedule appointments by Zoom or phone. A Zoom link will be sent to you prior to your appointment.

Continuity: If you previously worked with a DOS team member, please schedule with them.

Privacy: To keep your records confidential, and for appointment reminders, you must enter your AU email in the email field.
